The primary role of the Director of Social Work is to coordinate, supervise, and facilitate the delivery of Social Work Services, as provided by facility or facility contracted staff, to all residents of the Facility, and in accordance with applicable state and federal regulations and professional standards. This position fulfills the responsibility of direction of the facility’s social work services as required by the CT Public Health Code.
Responsibilities
Supervise the facility’s Social Work Services department and contracted staff consistent with operational guidelines.
Conduct face‑to‑face resident interviews and assessments of resident cognitive, mood, behavioral status in addition to other psychosocial factors.
Participate in planning for resident discharges.
Be available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed.
Direct and supervise the delivery of medically related social services to all residents of the facility, as well as their family members and significant others as needed.
Collaborate with the facility Staff Development Coordinator and Administrator to provide required and, as necessary, staff training in Residents Rights and Behavioral Health issues.
Participate in the clinical evaluation and screening of potential admissions to the facility.
Work with the facility’s interdisciplinary treatment team to access information regarding, and appropriately make referrals to, community‑based services for residents who wish to access such services.
Serve as a facility treatment team liaison with facility contracted mental health practitioners and consultants.
Actively participate in developing and implementing new behavioral and psychosocial service programs and presenting such programs to physicians, family members, and residents.
Qualifications
Master’s Degree in Social Work (MSW), preferred.
Bachelor’s Degree in Social Work or related human services field, required.
Minimum of one year’s experience in long‑term care providing direct social services, required.
Experience in Behavioral Health, preferred.
