Key Responsibilities
Oversee and manage the day‑to‑day operations of the social services department, ensuring high standards of care and service delivery.
Develop, implement, and evaluate social service programs to meet the psychosocial needs of residents and their families.
Provide counseling, support, and advocacy for residents facing adjustment, emotional, or discharge planning challenges.
Lead and participate in interdisciplinary care planning meetings, ensuring that social service goals are integrated into comprehensive resident care plans.
Facilitate resident admissions, transfers, and discharges, coordinating with residents, families, and external agencies.
Ensure the department's compliance with all federal, state, and local regulations pertaining to social services in long‑term care.
Supervise, mentor, and evaluate social work staff, fostering a collaborative and high‑performing team.
Develop and maintain strong relationships with community resources to facilitate appropriate referrals and support services for residents.
Address and resolve resident and family concerns, promoting open communication and satisfaction.
Qualifications & Experience
Master’s Degree in Social Work (MSW) from an accredited program required.
Current New Jersey license as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
A minimum of 3‑5 years of direct social work experience in a skilled nursing facility or long‑term care setting.
