is seeking a Social Services Director to provide comprehensive social services to residents and their families.
Position Summary Handles pressure, objectivity, confidentiality, organization, planning, reporting skills, persistence, proactive listening, verbal communication, and building resident and family relationships.
Essential Duties and Responsibilities
Establish a course of action by exploring options; setting goals with residents and their families.
Assist residents in achieving the highest practicable level of self‑care, independence, and well‑being.
Provide medically related social services to support residents’ physical, mental, and psychosocial health.
Evaluate social and family information and psychological/emotional needs to assess service needs and develop care plans.
Offer emotional support and address residents’ and families’ emotional problems, including coping with physical disabilities, health‑related fears, and grief.
Assist residents with health‑care decisions.
Assist with personal shopping, inventory, and tracking of patient belongings.
Coordinate responses to reports of missing, lost, or stolen belongings.
Secure resources from community and social‑services agencies to meet residents’ needs.
Obtain appropriate prosthetics and assistive devices.
Develop and maintain working relationships with other departments and community agencies.
Schedule outpatient appointments and on‑site ancillary services (optometry, podiatry, dentistry, psychiatric).
Assist with discharge planning and coordinate with interdisciplinary teams.
Provide in‑service training to staff as requested.
Counsel residents and family members.
Knowledge, Skills, and Abilities
Read technical procedures and policy manuals.
Present information effectively and respond to questions.
Apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Problem‑solve practical issues.
Interpret a variety of written and oral instructions.
Education & Experience
Bachelor’s Degree in Social Work or Human Services plus 2 years of supervised social‑work experience in a health‑care setting with direct resident contact.
MSW preferred.
Knowledge and experience with PCC preferred.
