Job Summary The Care Coordinator is a key member of our team who will deliver continuous patient monitoring and care coordination by assisting chronically ill patients from a remote setting. The Coordinator works closely with patients enrolled in Chronic Care Management (CCM) and/or Remote Physiological Monitoring (RPM) programs, communicating effectively and efficiently the patient’s care plan via telephone each month. The role supports patient education and collaborates with the broader care team to ensure seamless integration of services.

Qualifications
Certified Medical Assistant (CMA) or Registered Certified Medical Assistant (RCMA) or Medical Assistant (MA diploma) or Certified Nursing Assistant (CNA) from the State of Board of Nursing with active certification(s) required.
2+ years healthcare experience.
1 year acute care experience.
Call Center experience preferred.
Patient Care coordination experience in a Chronic Care setting is preferred.
Technology experience with multiple systems such as EMR/EHR, Microsoft Office, and comfortable learning new software platforms preferred.
Exceptional written and oral communication.
Outstanding interpersonal skills.
Strong work ethic, self‑motivated, organized, and able to work independently.
Ability to handle frequent interruptions and adapt to workload changes.
Ability to work or be available for full‑time work (36+ hours/week).
Bilingual (English/Spanish) required. Fluency in both languages required.
