Registered Nurse Case Manager
Location: Chesterfield, VA Salary: $72,466 – $97,827 Job Summary
As a Registered Nurse RN Case Manager, you assist patients in finding services needed for continuity of care upon leaving the hospital. Collaborate with community partners to provide services such as home health, hospice, skilled nursing, assisted living, private care, and durable medical equipment (DME). Responsibilities
Manage patient care from admission to discharge. Collaborate with the health‑care team and develop treatment goals and plan interventions to resolve psychosocial concerns. Document social work assessments, treatment goals, interventions, outcomes, and discharge plans. Provide program orientation to patients, families, and caregivers, including Medicare and insurance benefits. Coordinate patient/family/caregiver education and promote participation in team discussions. Ensure implementation of the treatment plan supporting patient strengths, abilities, needs, and preferences. Document findings of Discharge Planning Evaluation (DPE) and psychosocial assessments. Communicate needs and preferences to the patient/family/caregiver. Promote consistent, positive patient interactions that advance unparalleled patient service. Qualifications
Graduate of an accredited school of nursing (BSN preferred). Current Registered Nurse license in Virginia or multistate license. 3+ years of RN experience in a hospital setting preferred. Previous case management or utilization review experience preferred. Benefits
