Job Summary This position is responsible for facilitating smooth Home Care admissions, assessing and planning appropriate interventions to meet the clinical needs of patients and families prior to and upon admission to Blessing Home Care. The position works diplomatically and effectively across professional disciplines, particularly with the professional staff, as well as with patient/families, and outside agencies in accomplishing an acceptable outcome for the benefit of those served. This position requires providing services to one or more age populations, including infant through geriatric, in a manner that demonstrates an understanding of the functional/developmental age of the individual served.
Job Qualifications
Education/Training/Experience:
REQUIRED:
GRADUATE OF AN LPN SCHOOL OF NURSING
EXCELLENT WRITTEN AND VERBAL COMMUNICATION SKILLS
PREFERRED:
TWO YEARS NURSING EXPERIENCE
License/Certifications/Special Requirements:
LPN LICENSE IN ILLINOIS APPLY FOR MISSOURI LICENSE WITHIN ONE MONTH OF HIRE
