Care Plan Specialist

Position Summary: Eventus WholeHealth delivers an integrated model of care to adults who reside in skilled nursing, assisted living facilities, personal care homes, and independent living facilities through a network of healthcare providers including physicians, nurse practitioners, physician assistants, psychologists, licensed clinical social workers, and clinical and administrative support staff. In this role, the Care Plan Coordinator will be responsible for collaborating with providers in creating a comprehensive Care plan for use with Chronic Care Management (CCM) or Advanced Primary Care Management (APCM).
ESSENTIAL DUTIES AND RESPONSIBILITIES
Works directly with the billing/rendering providers to assist in creating a care plan for CCM/APCM use
Create care plans in conjunction with the providers to include medications, diagnosis, patient goals and interventions, care team members, barriers, patient education, and goals.
Track patients in need of care plans and follow up with provider(s) to complete care plans on a weekly basis.
Maintains knowledge of current health plan and agency requirements for creating care plans.
Maintains information in accordance with the guidelines for creating care plans.
EDUCATION AND/OR CERTIFICATIONS
High school diploma required
Associate Degree preferred
Skills And Qualifications
Prior experience in healthcare required; LPN experience a plus
Must be proficient in Microsoft Office with the ability to become proficient with company specific programs and software.
Strong organizational and time management skills, and ability to prioritize tasks and meet deadlines.
Professional verbal and written communication skills via phone, email, and other written correspondence.
Friendly and professional demeanor.
PHYSICAL REQUIREMENTS
While performing the duties of this job, the employee is regularly required to communicate with others, including the ability to express oneself and exchange information in what, at times, could be a stressful or high‑pressure environment.
The employee frequently is required to move about the office to access files, use office equipment, and to interact with others. The person must be able to sit or remain in a stationary position for extended periods of time to be able to work, communicate on the phone, utilize the computer, and interact with others. The employee is occasionally required to stand or otherwise be able to access files, equipment, and other items in high places.
Employee must be able to constantly operate computers, phones, and other office equipment.
Employee must be able to bend, stoop, kneel, crouch, or crawl.
WORK ENVIRONMENT
The noise level in the work environment is usually low to moderate.
Due to the collaborative nature of the business and the need to service customers, the employee must be able to interact effectively with others in an office environment, manage conflict, and handle stressful situations and deadlines.
Requires desk work in office environment.
Ability to work flexible hours as needed to meet deadlines.
