Base pay range
$45.00/hr - $50.00/hr
We are seeking a skilled and compassionate Home Infusion Registered Nurse to provide high-quality infusion therapy services to patients in their homes. This role is ideal for an independent, detail-oriented RN with strong IV skills who is passionate about patient education and continuity of care.
The Home Infusion RN delivers direct patient care under a physician’s plan of care while adhering to professional standards, regulatory requirements, and organizational policies.
Key Responsibilities
Perform initial patient evaluations, including physical and emotional assessments, home environment safety, and caregiver support
Establish, implement, and update individualized plans of care in collaboration with physicians and nursing leadership
Administer prescribed IV therapies, including starting and maintaining peripheral IV access
Educate patients and caregivers on infusion therapy, medication management, and safety protocols
Monitor for adverse drug reactions, side effects, allergies, and ineffective therapies, and communicate findings promptly
Coordinate care with interdisciplinary team members, including pharmacists and nursing leadership
Maintain accurate, timely clinical documentation and submit visit documentation within 72 hours
Participate in on-call rotation, quality improvement initiatives, and discharge planning
Supervise LVNs/LPNs where applicable and ensure compliance with agency policies
Adhere to infection control, safety, and confidentiality standards at all times
Minimum Qualifications
Active Registered Nurse (RN) license in the state of Vermont, in good standing
Minimum 1 year of nursing experience
Excellent IV start and infusion therapy skills
Current CPR/BLS certification
Valid state driver’s license with reliable, insured transportation
Ability to work independently in a home health environment
Strong written, verbal, and electronic communication skills
Schedule:
Monday–Friday, approximately 6 hours per day (30+ hours/week) with rotating on-call
