The primary purpose of your job position is to provide direct nursing care to residents and supervise day‑to‑day nursing activities performed by nursing assistants, ensuring compliance with federal, state, and local standards and regulations.

Delegation Of Authority As an RN you are granted the administrative authority, responsibility and accountability to carry out assigned duties within the RN scope of practice defined by the Ohio Nurse Practice Act/ORC Chapter 4723.

Risk Potential to Blood/Body Fluids:

Category I – tasks involving exposure to blood, body fluids, or tissues, requiring personal protective equipment.

Category II – tasks with no exposure but may require performing unplanned Category I tasks.

Category III – no exposure to blood, body fluids or tissues, and Category I tasks are not a condition of employment.

Administrative Functions

Direct the day‑to‑day functions of nurses and nursing assistants in accordance with regulations.

Ensure compliance of nursing personnel with written policies and procedures.

Review and recommend updates to policy manuals, job descriptions, etc.

Plan and conduct shift services, programs, and activities.

Maintain current Nursing Service Procedures Manual.

Submit reports and recommendations to the Director of Nursing.

Coordinate nursing services with other resident services and programs.

Ensure compliance with job descriptions and quality assurance program.

Participate in facility inspections and prepare units for surveys.

Review and update resident discharge plans and nursing plans.

Assist in planning nursing components of discharge plans.

Conduct weekly care conferences with physiatrist.

Interpret department policies and procedures for personnel, residents, visitors, and agencies.

Admit, transfer, and discharge residents as required.

Ensure accuracy of accident/incident reports.

Perform administrative duties including medical forms, reports, evaluations, and charting.

Charting and Documentation

Complete and file required record‑keeping forms upon admission, transfer, or discharge.

Review new admission charts for accuracy; report findings to the Director of Nursing.

Encourage physicians to review and sign treatment plans and progress notes.

Transcribe physician orders into resident charts.

Ensure nurse notes adhere to charting guidelines.

Document accidents/incidents involving residents.

Report discrepancies in physician orders, diet changes, or charting errors.

Complete transfer forms per procedures.

Record unauthorized discharges in medical records.

Sign and date all entries in residents’ medical record.

Drug Administration Functions

Ensure unit staff prepare and administer medications as ordered.

Maintain adequate floor stock of medications, supplies, and equipment; report needs.

Order prescribed medications, supplies, and equipment as required.

Maintain accurate narcotic records.

Notify Director of Nursing of drug or narcotic discrepancies.

Review medication records for completeness and accuracy.

Notify attending physician of automatic stop orders before final dose.

Dispose of drugs and narcotics in accordance with procedures.

Personnel Functions

Participate in performance evaluations and staffing recommendations.

Report staffing needs when personnel are absent.

Notify Director of Nursing of absentee call‑ins.

Evaluate workforce and recommend to Nurse Supervisor.

Develop work assignments and assist in implementation.

Provide leadership to unit nursing personnel.

Conduct daily rounds to ensure quality of nursing service personnel; report issues to Director of Nursing.

Supervise nurse aide trainees.

Meet with shift personnel to identify and correct problems and improve services.

Ensure personnel, residents, and visitors follow policies.

Build positive rapport with inter‑departmental staff.

Create a warm and calm unit environment.

Review complaints and grievances filed by personnel; report to Director of Nursing.

Administer disciplinary action fairly and without discrimination.

Exchange nursing reports upon shift change.

Report occupational exposures to blood and other hazards per policy.

Nursing Care Functions

Inform nursing personnel of new admissions and room assignments.

Ensure rooms ready for new admissions.

Participate in orientation of new residents and families.

Coordinate diagnostic and therapeutic services per physician orders.

Consult with resident's physician for care and treatment decisions.

Review resident charts for specific orders.

Implement and maintain nursing objectives and standards.

Ensure proper administration of prescribed treatments by nurses and CNAs.

Coordinate social and activity programs with nursing schedules.

Notify physicians of accidents or incidents.

Notify physicians and next‑of‑kin of significant condition changes.

Carry out restorative and rehabilitative programs.

Inspect nursing service areas for cleanliness and safety.

Administer professional services such as catheterization, tube feedings, suction, dressing changes, colostomy care, blood draws, massages, and range‑of‑motion exercises.

Use restraints when necessary per policy.

Collect specimens for labs.

Record vital signs, blood pressure, and other measurements.

Monitor seriously ill residents.

Check family‑brought food for dietary compliance and report concerns.

Ensure direct‑care personnel follow resident care plans.

Check residents unable to call for help frequently.

Meet with residents and families as needed; report problems to Director of Nursing.

Admit, transfer, and discharge residents.

Arrange transportation for discharged residents.

Escort discharged residents to pick‑up area.

Inform families of resident death.

Call funeral homes as requested; follow post‑mortem procedures.

Staff Development

Develop, plan, conduct, and lead in‑service training classes.

Maintain an effective orientation program for new employees.

Standardize work methods and train staff on blood/body‑fluid exposure.

Participate in outside training programs.

Attend annual OSHA and CDC in‑service training for hazard communication, TB management, and blood‑borne pathogen standard.

Participate in advance directive and community training.

Engage in continuing education to maintain license and knowledge.

Safety and Sanitation

Monitor personnel for compliance with safety regulations and equipment use.

Ensure adherence to dress codes and departmental policies.

Assist infection control in identifying and classifying routine work functions.

Provide training on exposure risks before tasks.

Maintain adequate supply of personal protective equipment.

Keep nurse stations and medication preparation rooms clean.

Maintain clean, safe, and sanitary resident care rooms and treatment areas.

Follow hand‑washing guidelines during nursing care.

Participate in fire safety and disaster drills.

Assist infection‑control program development and monitoring.

Follow isolation precautions when necessary.

Properly use and dispose of personal protective equipment.

Report hazardous conditions or equipment issues.

Ensure safe lifting and moving of residents with protective gear.

Report missing or illegible labels and MSDSs to the safety officer.

Equipment and Supplies

Recommend equipment and supply needs to Director of Nursing.

Maintain adequate stock of medications, medical supplies, and equipment.

Assist in safe operation procedures for nursing service equipment.

Ensure correct use of equipment.

Monitor usage to avoid waste.

Maintain MSDSs for hazardous chemicals.

Care Plan And Assessment Functions

Review care plans to ensure appropriate care is rendered.

Inform Care Plan Coordinator of necessary changes.

Ensure nurse’s notes reflect care plan adherence.

Assess resident care plans for goals and updates.

Ensure CNAs and nurses are aware of care plans.

Assist Coordinator in MDS, RAPs, and triggers revisions.

Resident Rights

Maintain confidentiality of resident care information.

Provide treatment with fairness, kindness, dignity, and respect.

Ensure privacy and knock before entering resident rooms.

Educate staff on resident rights, including refusal of treatment.

Report and document resident complaints; resolve per procedures.

Investigate allegations of abuse or misappropriation of property.

Respect resident refusal of treatment and advance directives.

Miscellaneous

Provide data to the Quality Assurance & Assessment Committee upon request.

Working Conditions

Work in office area and nursing service areas.

Education

Must possess a nursing degree from an accredited college or university.

Experience

On‑the‑job training provided.

Specific Requirements

Current, unencumbered, active RN license in this state.

Fluent in reading, writing, speaking, and understanding English.

Capable of making independent decisions when required.

Skillful in dealing tactfully with personnel, residents, families, visitors, agencies, and public.

Knowledgeable of nursing and medical practices, laws, regulations, and guidelines applicable to long‑term care.

Leadership and supervisory ability; willingness to work harmoniously.
