Description Provide support and instruction in activities necessary to promote and maintain independence and self‑determination for individuals with intellectual/developmental disabilities in a residential setting. Support trauma‑informed care and collaborate with program staff.

Position Details

Type: Part‑time (per diem)

Schedule: Nights, weekends, holidays

Location: 567 Holt Rd

Salary: $19.25 – $19.75 per hour (additional $0.75 per hour once certified to pass medication)

Essential Duties & Responsibilities

Provide person‑centered support, recognizing individual teaching and learning styles.

Offer direct personal assistance as needed.

Assist clients with health, wellness, meal preparation, shopping and medical appointments.

Communicate effectively with individuals, staff, supervisors.

Respond to incidents, provide first aid, handle medical emergencies.

Manage fiscal responsibilities, maintain ledgers, safeguard funds.

Maintain a clean, safe home following infection control procedures.

Communicate pertinent information shift‑to‑shift.

Attend staff meetings, promote client well‑being.

Maintain appropriate staff‑to‑client ratio.

Implement individual treatment plans.

Safeguard client valuables, document routine and unusual events.

Assist daily operations of the residence per regulatory and agency standards.

Follow mandated reporting procedures.

Maintain and expand skills through training.

Obtain and maintain all required trainings.

Ensure rights of individuals, treat with dignity and respect.

Transport individuals and business‑related travel, maintain agency vehicles.

Remain flexible with schedule, overtime, emergency shifts.

Participate in agency meetings and trainings.

Pursue professional development through reading, supervision, in‑service training, conferences.

Align with agency mission, values and strategic plan.

Other duties as assigned.

Additional Responsibilities for DSP II

Obtain and maintain AMAP certification.

Administer medications and personal care, document in Medication Management System.

Perform second medication checks.

Schedule and attend medical appointments as directed.

Qualifications

High school diploma or equivalent preferred.

Valid NY State Driver’s License; record must be acceptable to agency.

AMAP Certification required for DSP II.

At least one year of experience with individuals with developmental disabilities preferred.

Physical Demands / Work Environment Frequent sitting, standing, walking; lifting up to 25 lbs; requires close and distance vision; occasional noise.

Compliance Adherence to all federal and state laws and regulations, confidentiality, privacy, billing and documentation standards, and the CCFCS corporate compliance and ethics program.

Additional Requirements

Adherence to Direct Support Professional core competencies.

Strong verbal, written, basic math, computer skills.

Ability to teach, model and give support.

Willingness to work nights, weekends, holidays and travel.
