Hearts of the Rockies Home Health Care is seeking a compassionate and skilled
Physical Therapist
to join our growing home health team. This role is ideal for a clinician who values autonomy, meaningful patient relationships, and the ability to truly impact patients in their homes. You will perform in-person evaluations, recertifications, and discharges, while collaborating closely with our PTAs and interdisciplinary team.
Perform comprehensive
in-home evaluations
to assess mobility, strength, safety, balance, and functional limitations.
Develop individualized
Plans of Care
tailored to each patient’s goals and home environment.
Complete timely
recertifications and discharges , updating care plans as needed.
Care Coordination
Collaborate closely with PTAs, nursing, and the interdisciplinary team to ensure continuity and progression of care.
Provide ongoing clinical guidance and oversight to PTAs.
Participate in case conferences and communicate changes in condition promptly.
Patient & Caregiver Education
Teach home exercise programs and mobility strategies to improve function and safety.
Provide caregivers with practical education to support mobility, transfers, and fall-prevention.
Documentation & Compliance
Document all assessments and care plans in the EMR in compliance with Medicare, Medicaid, and agency standards.
Ensure all visits, goals, and outcomes meet regulatory requirements and clinical best practices.
Qualifications
Degree in Physical Therapy from an accredited program.
Active Colorado Physical Therapist license (required).
Strong understanding of anatomy, physiology, gait, balance, and functional mobility.
Home health or geriatrics experience preferred.
Ability to work independently while collaborating with a multidisciplinary team.
Excellent communication, problem-solving, and organizational skills.
