Physical Therapist – Home Health
Partners In Home Care (PIHC), Western Montana’s leading non-profit home health provider since 1987, is seeking a compassionate and skilled Physical Therapist to join our Home Health team. Conduct in-home physical therapy evaluations and develop individualized plans of care Provide skilled, evidence-based physical therapy treatments as ordered by the physician Adjust treatment frequency and duration based on patient progress and reassessment Complete and score OASIS assessments accurately and in compliance with Medicare regulations Maintain knowledge of Medicare Conditions of Participation and payer requirements Perform therapeutic interventions, including wound care when appropriate Document all care and communication using point-of-care electronic systems Coordinate daily schedules, participate in interdisciplinary care conferences, and support quality improvement initiatives Supervise Physical Therapist Assistants (PTAs) and CNAs as applicable Manage patient care episodes including admissions, re-evaluations, and discharges Maintain active Montana PT licensure, required continuing education, and ethical professional standards Participate in agency training and professional development activities REQUIREMENTS
Bachelor’s degree or higher from an accredited Physical Therapy program Current Physical Therapist license in the State of Montana (or ability to obtain) Valid driver’s license, reliable transportation, and appropriate auto insurance Access to high-speed internet Two or more years of physical therapy experience preferred Home health, outpatient, or clinical rehabilitation experience a plus Department:
