Licensed Vocational Nurse (LVN) – Hatcher Station Health Center – Full‑Time Primary Purpose:

Responsible for providing total nursing care for assigned patients, under the guidance of a Registered Nurse, to ensure quality care.

Qualifications

Graduate of an accredited school of vocational nursing and/or licensed by the Texas Board of Nursing as a Licensed Vocational Nurse or holding a valid Compact LVN license.

Current, valid LVN license (or temporary license) issued by the Texas Board of Nursing; or valid Compact LVN license.

Current healthcare provider BLS certification from the American Heart Association, American Red Cross or Military Training Network.

Current hearing and vision screening certificate within 90 days of hire.

Valid Texas Class C Driver License (required for School Based Health – Youth and Family Services).

Experience: 2 years healthcare experience preferred; 1 year of care coordination and planning, chronic disease management, population health, and/or telehealth preferred. No experience required for Virtual Care Only options.

Virtual Care Only:

Actively participates in proactive patient outreach using motivational interviewing to support health behavior change and self‑management.

Conducts comprehensive chart review to close gaps in care.

Assists patients and families with health system navigation, appointment scheduling, patient advocacy, determination of community resources, and electronic communication setup.

Skills / Abilities

Provides care to assigned patient population in accordance with the Texas Nurse Practice Act, established protocols, multidisciplinary plans of care, and clinical area standards.

Communicates effectively with all levels of staff, management, patients, and families.

Demonstrates patient‑centered behaviors and knowledge of appropriate nursing skills.

Responsibilities

Screens and prioritizes patients with regard to medical problems, ensuring appropriate treatment is delivered in a timely manner per physician orders and established policies.

Assists with inpatient/clinic coordination, patient flow, and discharge activities.

Maintains and implements knowledge of all rules, regulations, policies, procedures, guidelines, laws, and Joint Commission standards affecting nursing practices.

Identifies and meets educational needs of patients and families by communicating and distributing appropriate information, reinforcing teaching initiated by RN or NP, and may participate in community outreach activities.

Assists with precepting new employees and students as directed by supervisor.

Completes appropriate written and computer documentation according to established procedures and guidelines.

Consults with other team members, providing nursing care that demonstrates patient‑centered values while coordinating workflow and ensuring timely treatment.

Ensures supplies are available for individual patient care, gathering necessary items in advance and may assist with ordering and stocking.

Responsible for cleaning and sterilizing patient areas and medical instruments to maintain a safe environment.

Identifies ways to improve work processes and customer satisfaction, assists with developing annual goals and objectives for the nursing area, and implements results as appropriate.

Stays abreast of nursing advancements, attending seminars/workshops, reading professional journals, and maintaining LVN licensure; integrates knowledge into practice.

Sets up patient appointments and referrals for further testing and follow‑up care; assists in transporting patients as needed.

Virtual Care Only: conducts proactive patient outreach, comprehensive chart reviews, navigation assistance, and supports electronic communication design.
