Licensed Vocational Nurse (LVN) - Medical Infirmary Corrections FT Nights
Location: Lew Sterrett Justice Center, Dallas, TX.
Primary Purpose Responsible for providing total nursing care for assigned patients, under the guidance of a Registered Nurse, to ensure quality care.
Qualifications
Education:
Graduate of an accredited school of vocational nursing and/or licensed by the Texas Board of Nursing as a Licensed Vocational Nurse, or holder of a valid Compact LVN License.
Experience:
None (general). Virtual Care Only: 2 years healthcare experience preferred; 1 year of care coordination and planning, chronic disease management, population health, and/or telehealth preferred.
Certification/Registration/Licensure:
Current valid LVN license or temporary LVN license from the Texas Board of Nursing; or a valid Compact LVN license.
Current healthcare provider CPR completion card (American Heart Association, American Red Cross, Military Training Network).
Current hearing and vision screening certificate within 90 days of hire.
Skills:
Provide care according to the Texas Nurse Practice Act, protocols, multidisciplinary plan of care, and standards.
Communicate effectively with staff, management, patients, and families.
Demonstrate patient-centered behaviors and nursing skill competency.
Responsibilities
Screens and prioritizes patients with regard to medical problems, ensuring appropriate treatment per physician orders.
Assists with inpatient/clinic coordination and patient flow, including treatments as directed by physician and nursing staff and discharge activities.
Maintains knowledge of all rules, regulations, policies, procedures, guidelines, laws, and standards that impact nursing processes.
Identifies and meets educational needs of patients and families, communicates and distributes information, reinforces education initiated by RN or NP, and may participate in community outreach activities.
Assists with precepting new employees and students as directed by supervisor.
Completes appropriate written and computerized documentation according to established procedures.
Consults with other team members as required, provides nursing care that demonstrates patient-centered care, and anticipates and accommodates families, physicians, and staff needs.
Ensures supplies are available for patient care and may assist with ordering and stocking supplies.
Responsible for cleaning and sterilizing patient areas and medical instruments.
Identifies ways to improve work processes and customer satisfaction, makes recommendations to supervisor, and implements and monitors results as appropriate to department goals.
Stays abreast of latest developments, advancements, and trends by attending seminars/workshops, reading professional journals, and maintaining LVN licensure.
Sets up patient appointments and referrals for further testing and follow-up care.
Assists in transporting patients as needed.
Virtual Care Only:
Actively participates in proactive patient outreach, coaching patients, supporting health behavior change, and collaboratively setting goals.
Conducts comprehensive chart reviews for diagnostic findings requiring follow-up and closing gaps in care.
Assists patients and families with health system navigation, appointment scheduling, patient advocacy, and resource identification, and supports initiation of electronic communication modes.
