Seeking a Speech Language Pathologist for our Home Care team to provide individualized evaluation and treatment in patients' homes, focusing on communication, cognition, and swallowing to promote safety and independence.
Responsibilities include interpreting medical prescriptions, conducting assessments, developing evidence-based treatment plans, documenting progress, and educating patients and caregivers. Collaboration with interdisciplinary teams and participation in care planning are essential.
Qualifications: Master’s degree in Speech‑Language Pathology, CCC or CFY, NY State license, and CPR certification. Preferred experience with dysphagia and neurogenic disorders.
