The Advanced Practitioner will be a key member of an integrated Care Team which includes a Registered Nurse and a Social Worker.
We serve patients who struggle with multiple complex diseases, providing evidence‑based in‑home care and benefit management for those with multiple chronic conditions.

Roles And Responsibilities
Conduct assessments, including comprehensive annual wellness exams in the patient's home and virtually.
Counsel and educate patients and families about benefits and programs.
Document patient information, including chief complaint, diagnoses, history, review of systems, examinations, medications, allergies, assessment, and plan.
Coordinate care with primary care providers, specialists, and ancillary services.
Complete all documentation and paperwork in a timely manner.
Maintain quality of care standards as defined by the practice.
Work remotely in the state of employment, attending periodic training or meetings outside that state.
Deliver evidence‑based, timely care that reduces avoidable hospitalizations and maximizes quality of life.
Prescribe medications, order tests, and collaborate with Monogram physicians.
Performance measured and reported daily through quality outcomes.
Facilitate closing gaps in care by educating patients about preventive monitoring and scheduling diagnostic testing.
Assist patients with enrolling in educational videos.
Participate in integrated care team meetings.
Apply disease diagnosis and prevention knowledge.
Make assessments of patient health status, develop treatment plans, implement care plans, and follow up.
Other duties as assigned.
Position Requirements
Active and unrestricted Registered Nurse or Nurse Practitioner or Physician Assistant license.
Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.Ability to work autonomously without direct supervision.
Access to reliable transportation, valid driver's license, and car insurance.
Proficiency with required medical instruments and equipment.
Knowledge of computer‑based data management programs, information systems, and medical records technology.
Effective verbal and written communication with patients, families, physicians, and community partners.
Sound understanding of federal and state regulations including HIPAA and OSHA.
Minimum 2 years of direct patient care experience.
Experience in Managed Care/IPA/Health Plan.
Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home.
