Position Overview This position supports the PHP/IOP Outpatient Clinic. The registered nurse (RN, RN II, RN III) performs professional nursing activities in the care of patients, allowing them to achieve or regain and maintain maximum functional status. The role is governed by the Nurse Practice Act, the Administrative Code, and the professional standards for nursing practice, as well as the policies of Rogers Memorial Hospital.
Job Duties & Responsibilities Perform initial assessments and document as required.
Collect, record, and analyze pertinent data for admission:
Patient strengths and limitations that can be addressed to reach health goals.
Cultural, spiritual, and ethnic factors that may impact treatment.
Patient needs to be addressed at discharge.
Medical/physical status.
History of medication compliance, reactions, and current schedule.
Age‑specific data regarding the patient’s needs.
Involve patient's support systems (family, friends) in assessment and documentation:
Observe & document patient interaction with family/friends.
Obtain assessment data from support systems as appropriate.
Act as an advocate for patients:
Explain patient’s rights and obtain necessary signatures.
Provide information and obtain signatures on consents.
Use knowledge of patient rights to protect privacy and confidentiality.
Assist in patient orientation.
Apply unit safety policies, including gown & contraband search on admission.
Remain sensitive to patient/family stressors during admission.
Initiate and update treatment plan and documentation as required:
Participate in planning and modifying the patient’s plan of care.
Evaluate data, review treatment plan, and participate in care conferences.
Develop and interpret plan of care with patient/family.
Write clear, concise, achievable goals.
Review plan as goals change or are achieved.
On an ongoing basis, identify, interpret, and document information collected in nursing interview, observation, physical assessment and diagnostic data, and confer with other health care professionals as appropriate:
Review current lab data and follow up with doctor.
Evaluate fall risk and initiate precautions.
Identify potential self‑abuse, suicidality, or assaultive behavior.
Develop age‑appropriate interventions.
Assess changes in status and document interventions.
Implement patient care:
Demonstrate safe medication administration (right patient, medication, dose, time, route).
Maintain knowledge of medication purpose and effects; document appropriately.
Accurately transcribe and implement physician orders.
Monitor for adverse reactions, interactions, and other medication effects.
Conduct and document patient education about medications.
Stay informed about new pharmacologic products.
Identify potential patient care problems, abrupt changes, or impending instability and exercise leadership to intervene appropriately:
Use de‑escalation techniques (quiet room, seclusion, restraints).
Re‑evaluate safety level.
Recognize alcohol withdrawal syndrome.
Recognize neuroleptic malignant syndrome signs.
Recognize critical cardiac or respiratory symptoms.
Identify health education needs of patient/family for discharge:
Implement age‑appropriate teaching interventions.
Document in patient record and plan.
Organize care activities based on priorities and needs.
Implement care based on established policies and unit standards.
Ensure admissions, transfers, and related activities comply with EMTALA and regulatory requirements.
Adhere to nursing and hospital standards.
Protect against environmental hazards by following safety and infection control standards.
Participate in continuing education and in‑service programs.
Report education outcomes to unit staff.
Attend all scheduled in‑services.
Participate in the performance‑improvement program.
Assist in the development & implementation of unit standards of care (e.g., safety level, safety/hazardous items, Kardex):
Identify problems with unit systems, communication, and resources.
Suggest solutions to Clinical Services Manager.
Identify unit educational equipment and supply needs.
Serve on or lead unit projects.
Carry out leadership functions in patient care, staff operations, and department organization:
Contribute through committees and meetings.
Take initiative in evaluating and upgrading unit standards.
Assume charge nurse role:
Coordinate unit activities and manage crisis situations.
Facilitate admission process.
Communicate unit status and projected needs.
Maintain therapeutic milieu using clinical judgment.
Plan patient care assignments based on needs and staffing.
Keep unit orderly.
Model effective communication, teaching, and leadership.
Supervise & delegate to LPN/psychiatric technician.
Provide acuity monitors to Patient Accounts.
Maintain primary caseload and direct patient care.
Participate in projects, tasks, and continuing education opportunities to improve professional skills and unit/department systems:
Develop goals & objectives for professional growth and discuss with Clinical Services Manager.
Initiate continuing education to enhance skills.
Discuss progress with manager regularly.
Seek additional projects or tasks based on unit needs.
Promote department goals and the mission of the hospital:
Communicate goals to staff.
Demonstrate measurable goal achievement.
Maintain policies & procedures.
Include requirements of external agencies.
Communicate function backlog appropriately.
Educate new staff on regulations.
Train student interns as directed.

Education / Training Requirements
Bachelor’s degree in nursing (BSN) preferred. Registered nurse licensed by state board in the state of employment with license in good standing.
American Health Association Healthcare Provider CPR certification required within 30 days of hire. Bi‑annual recertification.
Formal training in aggressive patient management required within 60 days of hire. Annual recertification.
Previous psychiatric experience with children, adolescents, and adults preferred.
May be required to hold a qualified medical professional (QMP) designation per hospital bylaws.
Employees in Child/Adolescent Day Treatment programs must have either psychiatric nursing training or one year of clinical experience with these children.
