SUMMARY Provide care coordination services for the Minnesota Army National Guard in support of surge and contingency requirements as well as its steady-state mission. Contract personnel shall not perform inherently governmental functions (i.e., representation of government, decision authority, etc.) Responsible for tracking medical status of ALL assigned cases, or Soldiers as required.
This hourly position requires being on-site Mon-Fri and pays $29.65 per hour plus $4.93 per hour health and welfare (H&W) benefits. Please note H&W benefits are paid for a maximum of 40 hours per week.
DUTIES AND RESPONSIBILITIES
Track and manage Soldier medical cases from identification through medical readiness or administrative separation
Coordinate care plans with military and civilian medical providers
Prepare and process low-risk, low-acuity Line of Duty (LOD), IDES/MEB, and NDR-PEB cases
Receive, scan, upload, index, and maintain medical and administrative records in required systems
Update medical readiness, profiles, appointments, and case status in multiple tracking systems
Ensure all documentation and administrative charting is completed within required timelines (24 hours when applicable)
Track medical appointments and follow up with Soldiers and units
Maintain compliance with HIPAA, Army Regulations, and Individual Medical Readiness (IMR) requirements
Attend PHA, SRP, FRU, and deployment-related events as needed
Prepare required reports and provide documentation to government records custodians
REQUIRED QUALIFICATIONS
Proficiency in Microsoft Office (Word, Excel, Outlook)
Strong administrative, documentation, and organizational skills
Ability to handle sensitive medical information in compliance with HIPAA
Ability to manage multiple cases and meet strict deadlines
On-site Mon-Fri
PREFERRED QUALIFICATIONS
Experience with the U.S. Army or National Guard
Familiarity with military medical readiness or case management
Experience with systems such as MEDCHART, MEDPROS, eCase, HRR, MEB, or LOD tracking
