Overview Health & Human Services Agency in Lincoln, NE seeks a Service Coordinator to oversee Home and Community-Based Services (HCBS) waiver program for individuals with developmental disabilities. The role manages case management, assessment, and service coordination.

Responsibilities

Coordinate and oversee the delivery of services

Conduct assessments and eligibility determinations

Develop plans of services and supports

Provide appropriate referrals

Monitor provision of services

Ensure health, welfare, and safety of those we support

Qualifications Minimum Qualifications • Bachelor’s degree in education, psychology, social work, sociology, human services, or related field (or one year‑for‑year substitution). Consideration for graduates within 90 days of application. Experience in services or programs for persons with intellectual or developmental disabilities is required.

Preferred Qualifications • Experience developing person‑centered plans, completing assessments, habilitation, program implementation, team and meeting facilitation. Proficiency in Word, Excel, Outlook, Nfocus and Therap. Experience with special healthcare needs programs.

Other Valid driver’s license and reliable vehicle required. State vehicle may be provided depending on office location.
