The Home Services Social Worker identifies the psychosocial needs of patients and families through assessment. Social work interventions range from resource support identification and acquisition (including community support, financial and environmental enhancement) to short‑term counseling and emergent crisis intervention. Social services are provided as part of a collaboration with interdisciplinary teams. Hours may vary depending upon census and program need.

Qualifications Certifications: Proof of auto insurance – varies; Licensed Social Worker (LSW) – Illinois Department of Financial and Professional Regulation (IDFPR); Licensed Clinical Social Worker (LCSW) – Illinois Department of Financial and Professional Regulation (IDFPR); Driver’s License – Secretary of State (SOS).

Education: Bachelor’s Degree in Social Work; Master’s Degree in Social Work.

Work Experience: Hospice.

Responsibilities Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation. Capture all legitimate revenue; remain conservative in expense and resource use while providing appropriate service and supplies to patients. Develop a bereavement plan of care to address family member/caregiver needs. Interpret and communicate patient/family faith and cultural traditions. Identify and communicate when spiritual/religious beliefs may impact the physical and psychosocial care provided by other team members. Document recertification and discharge assessments that accurately reflect change in patient status and improvements realized relative to the initial assessment. Educate patients and family members in a manner that overcomes barriers, matches their learning capabilities, and meets fundamental needs. Investigate and apply payer‑specific commercial hospice benefits to optimize treatment opportunities. Complete visit notes and orders in a timely manner according to policy; correct care plans in a timely manner. Document psychosocial patient/family assessments, financial assessments, and MSW interventions within the patient’s electronic medical record. Implement a social work plan that results in: (a) enhanced family systems, (b) utilization of community resources by patients, families, and caregivers, (c) maximization of medical benefits, (d) enhanced environment for care delivery, (e) dignity for the dying patient, (f) maximized coping skills for patients and families, and (g) support for cultural beliefs and values. Evaluate the effectiveness of the plan of care and modify the intervention as indicated. Facilitate and support patient decisions and communication of self‑determined life‑care decisions. Provide care according to the plan of care/orders. Develop the social work plan in collaboration with the interdisciplinary team. Practice sensitively to the needs of patients and families; daily documentation reflects an understanding of palliative/comfort philosophy versus aggressive/curative treatment. Identify indicators of imminent death and address patient/family needs at the time of death. Demonstrate understanding of Medicare hospice benefit eligibility, admission qualification, election process, certification, recertification, transfer, non‑certification, and revocation. Utilize memorial funds appropriately and submit documentation in a timely manner according to policy. Demonstrate understanding of Medicare home‑care benefit eligibility, admission qualification, and services available. Complete or assist with advanced directives, including living will, HCPOA, and POLST forms. Assist with transportation barriers; refer for access to food, clothing, utility assistance; refer for Medicaid, community care, SSDI assistance; make elder abuse/neglect referrals.

Compensation and Benefits The compensation range for this position is $29.29 per hour – $50.38 per hour. This represents a good‑faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered to a candidate will depend on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked, and compensation model.
