Overview The Home Services Social Worker identifies the psychosocial needs of patients and families through assessment.

Social work interventions range from resource support identification and acquisition (including community support, financial and environmental enhancement) to short term counseling and emergent crisis intervention.

Social Services are provided as part of a collaboration with interdisciplinary teams. Hours may vary depending upon census and program need.

Responsibilities

Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.

Capture all legitimate revenue; be conservative in expense and resource use yet provide appropriate service and supplies to patients.

Develop a bereavement plan of care to address family member/caregiver needs.

Interpret and communicate patient/family faith and culture traditions, and identify when spiritual/religious beliefs may impact the physical and psychosocial care provided by other team members.

Document recert and discharge assessments which accurately reflect change in patient status and declines/improvements realized in comparison with initial assessment elements.

Educate patient and family members in a manner that overcomes barriers and matches their learning capabilities, meeting fundamental needs.

Investigate and apply payer specific Commercial Hospice Benefits to optimize treatment opportunities.

Complete visit notes and orders, and transmit them in a timely manner according to policy.

Correct care plans and transmit them according to the timeline.

Document psychosocial patient/family assessments, financial assessment and MSW interventions within patients’ electronic medical record.

Implement a social work plan that results in:

enhanced strength of family systems

patient/family/caregiver utilization of community resources

maximization of medical benefits

enhanced environment for care delivery

dignity for the dying patient

maximized patient/family coping skills

support for patient/family cultural beliefs and values

Evaluate effectiveness of social work plan of care and modify intervention as indicated.

Facilitate and support patient decisions and communication of self-determined life care decisions.

Provide care according to plan of care/orders.

Develop social work plan in collaboration with the inter‑disciplinary team (IDT).

Practice in a manner sensitive to the needs of patients and families. Daily practice and documentation evidence understanding of palliative/comfort philosophy and approach versus aggressive/curative treatment.

Identify and respond to indicators of imminent death, addressing patient/family needs at time of death.

Demonstrate understanding of Medicare Hospice Benefit including eligibility, qualification for admission, election process, certification, recertification, transfer, non‑recertification and revocation.

Utilize Memorial Funds appropriately and submit documentation in a timely manner according to policy.

Demonstrate understanding of Medicare Home Care benefit including eligibility, qualification for admission and services available.

Complete or assist and educate the patients/caregivers on advanced directives, including living will, HCPOA, and POLST forms.

Assist with transportation barriers.

Assist with referrals for lack of access to food, clothing, assistance with power bills.

Make referrals for help in the home.

Assist with applications (Medicaid, Community Care, SSDI).

Make Elder Abuse/Neglect referrals.

Compensation and Benefits The compensation range for this position is $29.29 per hour - $50.38 per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
