Catastrophic Nurse Case Manager Positions located in Chicago, IL and Northern Indiana. This is a home‑based role with up to 2 hours of travel to medical locations. The Case Manager will oversee medical case management of work‑related injuries: assessment, planning, coordination, implementation, and evaluation of injured or disabled individuals. The role engages insurance carriers, medical providers, attorneys, employers, and employees, monitors progress, and reports results to employers and carriers.

Key Responsibilities

Home‑based case management with up to 2 hours one‑way travel.

Assessment, planning, coordination, implementation, and evaluation of work‑related injury cases.

Collaborate with insurance carriers, medical providers, attorneys, employers, and employees.

Monitor progress of injured workers and report results to employers and carriers.

Qualifications

Current, unencumbered registered nurse (RN) license in IL.

Willingness to obtain IN RN license.

Experience in worker’s compensation catastrophic case management – preferred; may include critical care, catastrophic injury/illness case management, disability management.

Clinical experience in acquired brain injury, spinal cord injury, multiple trauma (including amputation), and/or burns.

Recommended certifications (CCM, CRRN, CDMS, CRC) – REQUIRED within two years of hire.

Ability to occasionally work outside traditional business hours (evenings or weekends).

Strong organization and timeline adherence, PC and technology proficiency, and written, verbal, and interpersonal communication skills.

Valid driver’s license and good driving history.
