Responsibilities
Perform patient start-of-care comprehensive assessments, develop and implement plans of care.
Complete OASIS assessments timely and accurately.
Provide skilled nursing care in accordance with the plan of care and agency policies.
Help achieve and maintain continuity of patient care by communicating with staff, physicians, family members and community resources.
Initiate appropriate referrals when indicated.
Promote and maintain patient health through teaching and appropriate rehabilitative measures.
Assist patients in learning appropriate self‑care techniques.
Perform other duties as assigned.
Qualifications
Current RN license specific to the state(s) you are assigned to work.
One year of clinical experience as an RN (less than one year requires approval by the VP of Clinical).
Current CPR certification.
Valid driver’s license, reliable transportation and liability insurance.
