Hours:
Every Saturday & Sunday Weekend option is a salaried position paid for 40 hours/weekend with on‑call requirement.
Must be available for orientation and training M‑F 8‑5 for the first 90 days.

Summary:
Description: The Home Health and Hospice Nurse, RN, Baylor is responsible for working from 5pm Friday thru 8am Monday and taking call as assigned during these hours administering skilled nursing care to patients requiring intermittent, professional nursing services to include Home Health and Hospice patients.
Qualification Requirements
Registered Nurse, currently licensed to practice nursing in the state of North Carolina. Minimum of one‑year experience as a professional nurse in an acute care setting.
Maintain current CPR certification (BCLS).
Previous experience working with terminally ill patients preferred.
Basic computer skills/knowledge and experience in electronic medical records.
Ability to understand and communicate well with patients, family, staff, physicians and the public.
Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public.
Must be competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patient's individualized plan of care.
Possess valid Driver's License and reliable transportation.
Essential Functions
Conduct admission of the patient, obtain medical history and assess physical condition, and develop a patient care plan in cooperation with other agency professionals.
Manage assigned cases and provide skilled nursing care as outlined on the physician's plan of treatment and in accordance with agency policies and procedures.
Observe signs and symptoms, report to the physician and other personnel, initiate preventative and rehabilitative nursing procedures, and ensure on‑going evaluation of patient/family response to care.
Adequately educate, teach, supervise, consult, and counsel the patient and family members regarding nursing care needs, disease process, self‑care techniques, end‑of‑life care, and ethical concerns.
Provide specialized hospice training to other staff, family members and informal caregivers to ensure adequate care.
Collaborate with supervisor to meet productivity standards, maintain up-to-date charting and reimbursement criteria, and perform weekend call and service training as assigned.
Assist with covering visits in other offices, maintain high degree of courtesy and professionalism, and follow agency personnel policies and compliance plan.
