Summary: The Home Health and Hospice Nurse, RN, Baylor is responsible for working from 5pm Friday thru 8am Monday and taking call as assigned during these hours administering skilled nursing care to patients requiring intermittent, professional nursing services to include Home Health and Hospice patients.
Qualifications:
RegisteredNursecurrentlylicensedtopracticenursinginthestateofNorthCarolina. Minimum of one-year experience as a professional nurse in an acute care setting.
MaintaincurrentCPRcertification(BCLS)
Previous experience working with terminally ill patients preferred
Basic computer skills/knowledge and experience in electronic medical records
Ability to understand and communicate well with patients, family, staff, physicians and the public
Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public
Must be competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patient’s individualized a plan of care
Possess valid Driver's License and reliable transportation
Essential Functions:
Conducts the admission of the patient to the Agency by: obtaining a medical history, to include Oasis when needed, particularly as it relates to the present condition, from the patient and / or familymember(s);performingaphysicalassessmentofthepatient;assessing thepatient'ssupport systems to determine what assistance will be available from family members and their ability to meet the patient’s immediate needs upon admission and throughout care; assessing the patient, familymember(s)andhomesituation to determinewhathealthteachingwill berequired;assessing the patient’s condition and home situation to determine if other home health services are needed; interpretingnursingandotherservicesoftheAgencytopatient'sandfamiliesasapartofplanning for care.
DevelopsandimplementsthepatientcareplanincooperationwithotherAgencyprofessionalsand paraprofessionalswhowillbeprovidingcaretothepatientin MatrixCare.
Managesassignedcasesandprovidesskillednursingcareasoutlinedonthephysician'splanoftreatmentandinaccordancewiththeAgency'spoliciesandprocedures.
Observes signs and symptoms and reports to the physician and other applicable personnel, reactions to treatments, including drugs and changes in the patient’s physical or emotional conditionandoutcomesofintervention.Ensureson-goingevaluationofpatient/familyresponse to care.
Adequately educates, teaches, supervises, consults, and counsels the patient / family members regarding the nursing care needs, other related problems of the patient at home, the disease process, self-care techniques, end-of-life care, and processes for dealing with issues of ethical concern.Alsoprovidesspecializedhospicetrainingtootherstaff,familymembersandinformal caregiverstoensureadequate care. careand safety.
EnsurethatcareisprovidedbytheHomeHealthNursingAssistantbyprovidingtheNursingAssistantwithawrittenplanofcareandspecificduties to be performed,bysupervisingandestimatingthecaregiven at least once every two weeks, andbyparticipating in periodic conferences with the Director of Clinical Practice or Assistant Director of Clinical Practice concerning the Nursing Assistant's performance.
Appropriatelyconsultswiththeattendingoroncallphysicianconcerningalterationoftheplanoftreatmentanddocuments,inwriting,appropriateschangeoforderswherenecessary.
Communicateswithsupervisorconcerningproblemsofthepatient(s)onatimelybasisandcommunicatesinformationusingcurrentprocessandtechnologyavailabletotheorganization.
Turnsinallpaperwork to the designated person in the office by 5pm on Monday, and syncs ipdad nightly when visitsaredone.Makesaverbalreportofnew admissions and ER visits to the primary nurse Sunday night or Monday morning between 8:00-9:00a.m.
Collaborateswithsupervisor to meet assigned productivity standards supervision.
Seesscheduledvisitsafter5:00p.m.onFridaysaswarranted. Worksatleastoneweekenddaywhenquality improvement/assurance activities, in-services, workshops, and seminars when appropriate.AdherestotheminutesoftheITCandstaff meetings.
Continuallystrives to improve nursing care through continuing education, active participation in professionalandrelatedorganizationsandindividualresearchandreadings.Evaluatesownneeds forsupportandusesidentifiedsystem(s)tomeetthose needs.
Attendstheappropriatehoursofin-servicetrainingperyearasrequired.
