Exciting Opportunity for a Case Manager in Eureka, California!
Position:
Registered Nurse - Case Manager
Experience Required:
2 years of acute care hospital case management experience
Shift:
Day 5x8-Hour (08:00 - 16:30)
Travel Experience:
Yes, first‑time travelers are welcome with manager's approval
Float Requirement:
Yes, based on departmental needs
Weekend Coverage:
Required every other weekend
Patient Ratios:
1:20
Daily Census:
Caseload up to 25
Key Responsibilities
Perform care coordination and discharge planning
Work with admission criteria, appeals, and denials
Assess benefits eligibility and conduct needs assessments
Handle prior authorizations and DME orders
Collaborate with interdisciplinary support including social services
Skills & Requirements
Experience with HMOs, IPAs, and advance directives
Familiarity with regulatory guidelines (CMS, HIPAA, The Joint Commission, etc.)
Strong understanding of worker's compensation case management
Passion for helping diverse patient age groups: adolescents, adults, pediatrics, geriatrics
