Position: Case Manager
Profession: Registered Nurse
Specialty: Case Manager
Shift: Day 5x8-Hour (08:00 - 16:30)
Weekend Requirement: Every Other Weekend
Key Responsibilities
Provide exceptional case management services in an acute hospital setting.
Conduct care coordination and discharge planning for diverse patient populations.
Manage a caseload of up to 25 patients with interdisciplinary support.
Utilize charting systems, specifically Epic, for accurate patient documentation.
Float between like units as needed to support departmental needs.
Qualifications
Minimum 2 years of acute hospital case management experience with a focus on care coordination and discharge planning.
Experience with HMOs, IPAs, advance directives, POLST, and protective services process.
Strong understanding of regulatory requirements and guidelines, including CMS, DRG, and HIPAA.
Previous experience in related acute hospital specialties preferred (Emergency Department, ICU, etc.).

Patient Ratios: 1:20 during the day shift.
