Position Summary: In compliance with Federal Law, all persons hired will be required to verify identity and eligibility to work in the United States and to complete the required employment eligibility verification form upon hire. Working under general supervision and with moderate difficulty, this position assists and implements activities that support the Family Caregiver Support Program (FCSP).

Salary and Hours $21.59/hr, 40 hrs/week, 52 weeks/year

Essential Duties And Responsibilities

Identify and assist with intake of new caregivers to provide or refer needed services.

Assist with outreach and development of outreach materials to provide caregivers and caregiver support groups information and access to services.

Assist with set up and running of caregiver support groups, events, programs, and workshops to ensure caregivers are receiving access and needed support to the program.

Authorize respite units in the state database.

Provide support to the Care Coordinators when an identified family caregiver is in crisis.

Other related duties as assigned.

Education And/Or Experience High School Diploma / GED and two years’ experience in health, social services, or customer service; OR an equivalent combination of education, training, and experience which demonstrates the ability to perform the duties of the position.

Physical Requirements & Work Environment Physical Activity: Frequent reaching, standing, sitting, walking, finger use, grasping, talking, hearing, and repetitive motion. Occasional climbing, balancing, stooping, kneeling, crouching, pushing, pulling, lifting, and feeling.

Physical exertion: Sedentary; Light; Medium; Heavy; Very Heavy. Work involves exerting up to 25 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.

Visual requirements: Close visual acuity for compiling and analyzing data; viewing a computer screen; and reading.

Working conditions: Occasional exposure to indoor and outdoor environmental conditions and infectious disease.
