Scope of Role
Responsible for providing direct support within a single program or unit, typically supporting 4–6 individuals in residential settings or up to 10 individuals in periodic programs.
Key Responsibilities
Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, and skill/behavioral development.
Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
Essential Job Functions To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.
Services and Supports
Direct support or assistance in accordance with individual plans; assists with socialization, behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living.
Accurately documents progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.
Makes cost‑effective purchases within the budget of the individual served, respecting their choice and preference.
Maintains healthy professional relationships with individuals, families, guardians, and case managers; greets family members and other visitors; implements Sevita Customer Service Standards.
Reports any instance of alleged abuse or neglect, medical, behavioral, and other incidents in accordance with Sevita policy and external requirements.
Maintains confidentiality, respects the rights of persons served, applies the bill of rights, practices universal precautions, and assists individuals in exercising their rights.
Health Care
Accompanies individuals to medical appointments as needed, relaying orders and information as required.
Accurately administers or supports self‑administration of medication when assigned; documents delivery and promptly reports errors; maintains security of controlled medications and supplies.
Monitors individuals’ health, documents concerns, communicates with nurse or supervisor, calls 911 in emergencies, and follows health directives.
Follows procedures for medical equipment and reports malfunction or disrepair.
Prepares or supports preparation of meals following planned menus and individual choice; knows special diets; monitors mealtime and implements dining plans.
Employment Responsibilities
Attends orientation and ongoing training; participates in monthly staff meetings.
Works scheduled hours; arrives on time; stays on shift until replacement arrives; notifies supervisor in advance when unable to work.
Works cooperatively with others, values contributions, is courteous, communicates openly, listens, and shows respect.
Maintains a safe workplace, complies with safety policies and procedures, and reports hazards to supervisor.
Maintenance
Transport individuals into the community; drives safely, uses safety equipment, and reports accidents immediately.
Performs daily and seasonal housekeeping, maintains a neat, clean, and safe environment, and reports hazards.
Monitors environmental safety, makes minor repairs, and performs yard maintenance as needed.
Checks water temperature during bathing, participates in safety drills, and protects individuals during emergencies.
Other
Performs additional duties as required.
Uses Mobile Punch for timekeeping.
Supervisory Responsibilities
None
Minimum Knowledge and Skills Required Education and Experience
High school diploma or equivalent preferred.
Six months of experience in human services preferred.
Must be 18 years of age.
Working knowledge of computers.
Certificates, Licenses, and Registrations
Valid driver’s license in good standing.
Car registration and vehicle insurance if providing transportation.
All state‑required training(s) and certification(s) completed in mandated timeframes.
Maintain valid driver’s license, meet insurance requirements, and be approved as a company driver; non‑driver status if driving privileges are lost.
Work Environment
Works at program location and may accompany individuals into the community.
Physical Requirements
Heavy work: exerting up to 100 pounds occasionally, up to 50 pounds frequently, and/or up to 20 pounds constantly to move objects.
Addendum
Employees must cooperate with the licensee and Department of Human Services staff in any inspection, inquiry, or investigation.
Must have a valid driver’s license in good standing in the state of residence; if residing outside New Jersey, meeting that state’s licensing requirements.
At minimum, a High School Diploma/Equivalent is required.
All employees hired after 6/15/2018 shall submit to pre‑employment drug testing; random testing thereafter.
Employees must submit to criminal background checks and be cleared by the Department of Human Services before working with served individuals; must also be checked against the Central Registry of Offenders Against Individuals with Developmental Disabilities and the Child Abuse Registry Information.
Employees must successfully complete and demonstrate proficiency in all required trainings.
Shall be at least 18 years of age.
