RN Case Manager I-HC We are seeking a dedicated RN Case Manager to provide comprehensive nursing care for a set of assigned patients. The role involves assessment, planning, coordination of interdisciplinary services, and supervision of Private Duty Nurses (PDN) as applicable.

Schedule 40 hours per week

Attributes

1 year of nursing experience preferred.

Prior home health, clinical and direct patient care experience preferred.

Ability to work independently.

Solid interpersonal, organizational and time management skills.

Proficient computer skills.

Must be able to document clinical notes and assessments within an electronic medical record.

Ability to travel to required locations as needed.

Responsibilities

Identify and prioritize health problems based on assessment.

Develop or implement an interdisciplinary Plan of Care based on identified needs, with patient input and collaboration with the attending physician and other team members.

Manage and coordinate patient care, including clinically complex cases, ensuring efficient and effective delivery of services and community supports.

Exhibit proficiency and accuracy in comprehensive assessment/documentation, including payer-required assessments such as OASIS and HIS.

Minimum Qualifications

Diploma or Associate’s Degree in Nursing required; Bachelor’s Degree preferred.

Additional Qualifications

Level II: a minimum of 2 years of home care experience.

Education & Licenses Licensed RN; certifications as required by department.

Physical Requirements M - Medium work: exerting 20‑50 pounds occasionally, 10‑25 pounds frequently, or >10 pounds constantly; requires frequent walking, standing, or squatting.

Pay Range $77,983.00 – $103,906.00 per year.
