About the Position Job Title: Physical Therapist (PT) Home Health Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 year of adult home health experience License: Must have a valid license in hand Certifications: CPR, BLS Description: Home health visits will be conducted. Periodic meetings may be required. Must have reliable transportation and a current driver's license. Must maintain agency-required liability insurance. Average number of patients seen per day is 5-6. Contractor will be assigned between 5-10 patients per day. This equates to roughly 6-8 points of productivity. Guaranteed hours are not provided. In case of low patient census, contractors should discuss options for floating to nearby clinics. Types of cases include start of care and follow-up visits. First-time applicants are not accepted; recent home health experience is required. Weekend work may vary depending on location and profession; to be discussed during the interview. On-call requirements vary by job and must be discussed during the interview. Willingness to travel up to 75 miles from the home clinic is necessary. Mileage will be reimbursed at the IRS standard rate. Compensation is provided from patient home to patient home. The commute to and from the first and last patient is not billable. Local candidates are accepted at the same rate. Pending licenses are not accepted; candidates must have their license in hand unless it is taking a few days to obtain. Weekly expectations of productivity are 30 points, which equates to approximately 40 hours of work. All documentation must be completed within 24 hours of the patient appointment. Failure to complete documentation on time may result in a cancellation. There are no guaranteed hours for this contract. The clinician must accept every patient on their schedule, regardless of distance.
