Occupational Therapist

Golden Glades Nursing & Rehabilitation Center - To provide occupational therapy services to the resiident in accordance with Florida State pratice standards in accordancewith the American Occupational Therapy Association.

Certification: Florida State License and current Florida Registration Certificate

Preferred Experience: 2 years of experience, at least one in Geriatrics

Job Duties

Performs initial screens/evaluations to determine needs for skilled services.

Outline, implements, and modifies residents' specific treatment plan.

Performs treatment sessions striving toward functional resident specific treatment plan.

Documents functional progress and needs for continuation of skilled services.

Maintains daily statistic records in Net Health

Recommnds to DOR termination of OT services when approriate.

Provides in-service education to nursing staff and family members as needed.

Communicates effectively with other therapists & departments re: common issues(discharge plans & significant changes) including Social Services, Nursing, Admissions, MDS Coordinator, & Housekeeping.

Aware of state requirement Continuing Education need as per State Policy.

Attends all department & interdisciplinary meetings as scheduled.

Complies with HIPAA guidelines.
