Overview Make a lasting difference as a Hospice Registered Nurse Case Manager (RNCM)! Sanctuary Hospice is seeking a Full‑Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.

Schedule:

Monday – Friday, 8 am – 4:30 pm and On Call As Needed. Territory:

Rockford and surrounding counties – Winnebago, Boone, McHenry, Dekalb, Ogle, Stephenson, and Lee.

Responsibilities

Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.

Partner closely with our Clinical Director to assess eligibility and suitability of clients for specialized hospice services.

Provide comfort and support to patients, ensuring their comfort and quality of life remain paramount.

Deliver top‑notch nursing care rooted in proven principles and techniques, easing pain and bringing solace.

Educate and empower patients and families through personalized care plans.

Share call duties as part of the team’s commitment to being there when it matters most.

Qualifications

Current, unencumbered license as a Registered Nurse.

Minimum one year of RN experience in a medical‑surgical/acute care setting; hospice experience preferred.

Computer proficiency, including EMR background.

Current CPR certification if required by state.

Valid driver’s license, vehicle insurance, and dependable transportation.

Compensation $89,000 – $96,000 per year (Average Pay Range) . The pay range listed is a general guideline and is not a fixed offer. Final compensation will be determined based on experience, qualifications, and specific responsibilities and will be discussed at the conclusion of the interview process.
