Position Summary A Social Worker is a licensed professional trained to provide social support services as outlined by agency programming and policies. Social Workers report directly to the Director of Services and work to advocate for the psychosocial, financial, social, emotional, and mental health needs of children and young adults diagnosed with a chronic and/or life‑threatening illness as it relates to their health status and on‑going treatment.
Minimum Requirements
Bachelor of Social Work (BSW) – Clear Background Check
State of Nevada Licensed Social Worker – 2‑year min. social work experience preferred

Primary Duties & Responsibilities
Integrate emotional, psychological, financial, and educational needs of patients and families with medical interventions
Coordinate and follow up on client referrals originating from clinics, hospitals, self‑referrals, and other community‑based referral sources to ensure timely engagement in services
Meet with families and complete psychosocial assessment
Facilitate activity workshops for children, siblings, and caregivers
Assist in the navigation of the health care system (social security disability insurance & Medicaid)
Advocate for the appropriate care of children
Offer coping skills to manage ramifications of disease for patients, survivors, and those who suffer secondary effects of illness
Research community and national organizations identifying aids and resources
Work within the scope of practice as outlined by professional standards, ethical practice, and agency policies
Provide people, families, or vulnerable populations with the support needed to cope with chronic, acute, or terminal illnesses, such as cancer, HIV/AIDS, sickle cell, renal, blood and immunologic diseases
Advise family caregivers, providing patient education and supportive services
Prepare individualized intervention plan and make social services referrals as needed
Build client charts (electronically), file and manage accordingly
Maintain active case management and complete required documentation of client contacts and services provided in a timely manner
Facilitate NVCCF program assigned and report results
Facilitate internal referrals for services provided within the agency
Work side by side with NVCCF staffers and medical professionals to ensure client needs are fulfilled
Provide feedback on effectiveness and/or efficiency of programs and services
Attend staff meetings and case staffing to stay current of agency and client updates
Maintain current knowledge base on social work issues through conferences, educational requirements for licensure, and social work literature
Use and apply new and changing technical requirements as directed
Develop specific goals and plans to prioritize, organize, and accomplish work
Perform miscellaneous job‑related duties as assigned
Work after business hours, weekends and during foundation events/programming to accommodate various programs and services
Provide agency services in an outside acute or sub‑acute setting if needed
May be required to follow hospital/clinic volunteer protocols such as drug screening, background checks, tuberculosis testing, & vaccinations
