As our Referral Care Coordinator, you will be responsible for managing the process of receiving, scheduling and triaging calls and referrals from the community, healthcare providers, and other internal and external referral networks. This position serves as a valuable first point of contact, primary source of support and education, and helps ensure a full understanding and navigation of SouthLight's life‑saving services. The Referral Care Coordinator also supports the Customer Care Coordinator in receiving, screening and directing a high volume of external and internal calls received on SouthLight's main phone number.

Schedule Monday‑Friday 8am‑4pm.

Pay $18.00 per hour.

Full‑time employees are eligible for medical benefits the first of the month, 30 days after hire. Options include a standard plan and an HSA plan with a $300 initial contribution and up to a $25 per paycheck match. Employees receive 10 paid holidays and PTO that accrues at 10 hours per month. After 6 months, employees may enroll in our 401(k) plan, which offers a 4% match. Additional offerings include company‑paid life insurance, short and long‑term disability, in‑house and paid training opportunities, and extra pay for working holidays.

Job Responsibilities

Provide superior customer service when communicating in person, via phone, or online with staff, service providers, hospitals, and the general public.

Answer incoming calls, provide information about treatment services and general inquiries, and transfer calls to the appropriate source as needed.

Oversee and manage online referral portals from hospitals and service providers, and coordinate services appropriately.

Request and review required documentation to verify compliance, completeness, and data accuracy.

Respond to all referral requests within 24 business hours of receipt and document outcomes.

Utilize the electronic medical record to establish, update, and maintain accurate client information.

Develop active knowledge of available resources and work to build and strengthen professional relationships with community providers.

Qualifications Education and Experience

Minimum of a high school diploma or GED.

Minimum one year of experience in a front office, customer service, or related field; behavioral health experience is a plus.

Knowledge, Skills and Abilities

Demonstrated ability and willingness to learn, strong attention to detail, a positive attitude, and flexibility to embrace change in a fast‑paced environment.

Commitment to strong customer service focus rooted in empathy and connection with clients.

Excellent time management and prioritization skills to handle a high volume of inquiries.

Professional interpersonal and communication skills.

Ability to work independently and as part of a team.

Organizational skills to prioritize workload, meet deadlines, and carry out project assignments efficiently while providing accurate documentation of activities.

General understanding of MCOs, Medicaid, Medicare, and third‑party payers.

Ability to interact effectively with persons of diverse backgrounds, cultures, and socio-economic classes, including those in crisis or resistant to the organization.
