Scheduled Weekly Hours: 40
Hours:
Monday – Friday, 8:00 am – 4:30 pm
Job Summary Works in collaboration with a physician and the Transitional and Primary Care team to serve as an integral part of the post‑acute and primary strategy to reduce hospital admissions, readmissions, and potentially preventable emergency visits. Provides education and establishes goals for patients and families within the framework of the Louisiana Nurse Practice Act.
Minimum Qualifications
Master’s degree in Adult Health Nursing/Nurse Practitioner.
At least two years’ experience in an acute care setting, clinic setting, and/or home‑based setting.
Current Louisiana APRN license.
Current Basic Life Support (BLS) certification from an ILCOR‑approved course (e.g., Ochsner Health System, American Heart Association, American Red Cross) including a manikin check‑off.
Valid, state‑issued driver’s license and reliable transportation with current safety inspection, vehicle registration, and proof of automobile insurance that meets minimum state liability limits.
Clear driving record: no suspended or revoked license, no more than three moving violations in the past 36 months, no more than two accidents in the past 36 months, no DUI/DWI in the past 24 months, not at fault in a fatal accident in the past five years, no abandonment of an accident scene in the past 36 months, and no reckless driving in the past 12 months.
Preferred Qualifications
Experience in palliative care or hospice.
ACLS and/or PALS certification.
Physical Demands Must possess good physical health and be capable of standing, sitting, or walking for extended periods, lifting up to 50 pounds, and working in a moderate noise environment.
Physical Effort Required:
Constant (67%-100%): handling/feeling, talking, hearing, seeing.
Frequently (34%-66%): lifting, carrying, pushing/pulling, stooping, crouching, reaching.
Occasionally (1%-33%): climbing stairs or ladders, balancing.
