Responsibilities
Provide outpatient rehabilitation services in home or independent living facility settings for older adults, evaluating, planning, and delivering individualized therapy programs.
Supervise PT assistants, temporary licensees, students, and support personnel in accordance with state licensure and professional standards.
Maintain accurate clinical and billing documentation, adhere to treatment schedules, and participate in patient care and staffing conferences.
Make referrals to other disciplines and agencies to address comprehensive patient needs.
Consult with rehabilitation team members to select appropriate therapeutic regimens based on patient capabilities.
Adapt treatment programs and plan of care to individual patient needs.
Promote a safe environment by enforcing safety protocols and eliminating hazards.
Order, maintain, and report needed supplies and equipment.
Clean and maintain equipment, reporting repairs as necessary.
Ensure patients and families receive high‑quality, compassionate care that recognizes individual needs and rights.
Screen, examine, and evaluate patients (history, systems review, tests) to diagnose, identify impairments, establish goals, and design plan of care, coordinating with caregivers.
Design and implement treatment plans, including therapeutic exercise, functional training, manual therapy, airway clearance, integumentary care, modalities, health and wellness, chronic disease education, falls assessment, case management.
Fabricate, fit, and train patients on adaptive devices (orthotics, prosthetics).
Select activities suited to patient capacity and interests, instruct skills, and evaluate progress, attitudes, and behavior.
Perform routine re‑examination to modify or progress treatment plan.
Incorporate health and wellness programming into treatment plans.
Perform home and community mobility assessments, providing recommendations for safe mobility.
Perform other related duties as required.
Qualifications
Valid Physical Therapy license in the state(s) of practice or proof of license eligibility.
Master's degree in Physical Therapy or a related field with a Bachelor's in PT.
Alternatively, Bachelor's in PT with 5 years of progressively responsible PT experience.
Qualifying PT degree earned outside the U.S. must be evaluated as U.S. equivalent.
Valid driver's license or reliable independent transportation to patients’ residences.
Posted Salary Range USD $45.00 – $61.00 per hour.
Bonus USD $5,000 bonus.
