Responsibilities
Primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population.
Assess and develop physical therapy programs to facilitate rehabilitation.
Make referrals to other disciplines to address the comprehensive needs of patients.
Implement individualized therapy programs designed to restore, reinforce, and enhance performance.
Instruct, educate and train patients and caregivers in skills and functions essential for independence and productivity.
Direct activities to promote and maintain health.
Supervise Physical Therapy Assistants, temporary licensees, students and support personnel as required.
Ensure adherence to department and patient treatment schedule, timely clinical and billing documentation.
Attend and contribute to patient care, staffing conferences and related meetings.
Consult with other rehabilitation team members to select appropriate therapeutic regimens.
Adapt program and/or plan of care according to individual needs.
Promote a safe environment and enforce elimination of fire and safety hazards.
Order and maintain supplies and equipment; report repairs when necessary.
Screen, examine and evaluate patients; develop treatment goals and methods; implement treatment plan; complete documentation and record keeping.
Apply therapeutic techniques such as therapeutic exercise, functional training, manual therapy, airway clearance, integumentary repair, electrotherapy, physical agent and mechanical modalities.
Provide education on chronic disease management, falls assessment and interventions to reduce fall risk.
Perform home and community mobility assessments and provide safety recommendations.
Perform routine reexamination as needed to modify plan of treatment.
Incorporate health and wellness programming into treatment plans.
Perform other related duties as required.
Qualifications
Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
Master’s degree in Physical Therapy or a related field in addition to a Bachelor’s degree in Physical Therapy, or a Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
Qualifying PT degree earned outside of the U.S. must be evaluated as equivalent to a U.S. PT degree.
Valid driver’s license or reliable, independent transportation to patients’ residences.
Posted Salary Range USD $52.00 – $65.00 /Hr. Bonus: USD $5,000.00
