Responsibilities
Provide medical management for seriously ill patients, addressing symptoms and aligning treatments with goals and values. Collaborate with interdisciplinary clinicians to deliver the best care at home or in facility settings. Lead palliative and hospice care services, including patient‑family education and support. Participate in community outreach and serve as a regional medical community member. Qualifications
A heart for patient care and a passion for the best possible outcomes. Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing. At least 2 years’ experience in chronic disease management/long‑term care; hospice experience preferred. Reliable transportation and ability to sit, stand, bend, move intermittently and lift 80–100 lbs. Clinically competent and capable of caring for patients at end‑of‑life. Benefits
