Monday – Thursday, 2:00 p – 12:30 a Responsibilities
Assume responsibility for patient visits or on‑call schedule, including assessment, planning, implementation and evaluation phases of the nursing process Provide hospice nursing care based on systematic assessment, focusing on pain control and symptom management Initiate communication with attending physicians, hospice staff and other agencies as needed to coordinate care and use resources for the patient/family Maintain regular communication with direct supervisor to review assignments Maintain up‑to‑date documentation at point of service so that problems, plans, actions and goals are accurately and clearly stated and charges are reflected as they occur in accordance with agency policy and Medicare regulation Administer medication in accordance with all federal and state laws and the policies of the Hospice along with LTC staff Maintain current patient records and documents in accordance with the policies of Ascend Hospice & Palliative Care KS to ensure that problems, interventions, goals and outcomes are accurately and clearly stated and charges are reflected as they occur Assess and collaborate with Medical Director to determine initial and ongoing eligibility of the hospice patient Complete an initial assessment of patient and family to determine home care needs Anticipate end‑stage disease process and related pain and symptom management Regularly re‑evaluate patient’s nursing needs Use assessment data to determine care plan needs Initiate appropriate preventive and palliative nursing procedures Counsel the patient and family in meeting end‑of‑life needs Perform a comprehensive death note when appropriate Communicate with the physician regarding the patient’s needs and report any changes in the patient’s condition Communicate with other members of the interdisciplinary team to coordinate the care plan Participate in on‑call duties as defined by the on‑call policy Ensure arrangements for equipment and other necessary items and services Qualifications
Associate’s Degree in Nursing State‑certified RN license Experience in hospice or home health care (REQUIRED) Excellent documentation skills and ability to work independently Strong communication skills Valid driver’s license and reliable transportation Available to travel locally within assigned territory Benefits
