Care Coordinator (OhioRISE)
Join Us: The Integrated Services for Behavioral Health (ISBH) is a community‑mindful, forward‑thinking behavioral health organization serving southeastern and central Ohio with a comprehensive array of behavioral health and other services. We meet families in their homes and communities to connect them with needed resources, ensuring collaborative and personalized care.
Location:
Washington County, OH
Pay:
$20.19 – $25.03 per hour (based on experience, education, and/or licensure)
Essential Functions
Partner with families to identify care coordination needs and services that align with program outcomes.
Work with families to identify cultural factors influencing strengths, functioning, and interaction styles for ongoing engagement.
Identify strengths of children, youth, and families to support healthy outcomes.
Co‑ordinate family‑based services in home, school and community settings.
Ensure services on the care plan are appropriate, least restrictive, and meet safety & treatment needs.
Build positive relationships with children, youth, families, and child & family teams, supporting integration of care plans.
Develop collaborative partnerships with community resources to meet diverse needs.
Maintain documentation, participate in program evaluation, attend meetings, cross‑systems training, and stay knowledgeable about community resources.
Complete required training including High Fidelity Wraparound, MI, Cultural Humility, etc.
Perform any other duties as assigned.
Minimum Requirements
Experience providing services to children and families in behavioral health, child welfare, developmental disabilities, juvenile justice, or a related field:
3 years with a high school diploma or equivalent;
2 years with an associate/bachelor’s degree;
1 year with a master’s degree or higher.
Knowledge and experience in Hi‑Fidelity Wraparound preferred (certification provided at employment).
At least 2 years in a coordinated supportive services or care coordination role preferred.
Experience working with autism spectrum disorders and developmental disabilities preferred.
Additional preferred experience areas:
Family systems
Community systems & resources
Case management
Child & family counseling or therapy
Child protection
Child development
Managing complex cases with cultural humility or responsiveness
Experience with SED, trauma, co‑occurring behavioral health disorders in child‑serving systems
Strong organizational skills and ability to prioritize multiple tasks
Cultural awareness
Multi‑need individuals/families experience
Broad knowledge of community service systems
Willingness to engage in cross‑systems care coordination
Effective verbal/written communication
Internet‑based office proficiency
Valid driver’s license
