BayCare Health Systems is currently seeking an experienced Case Manager to join our outstanding and compassionate team in Hernando County! This is a field‑based position that works throughout all of Hernando County.
Location and Availability
Location: Brooksville, Hernando County
Status: Full‑Time, Hourly
Shift Hours: Vary

Weekend Work: No

The Case Manager Is Responsible
Case Managers provide direct behavioral case management services to adults or children with serious mental health conditions, substance use disorders, and/or those whose involvement in the child welfare system requires.
The Case Manager should demonstrate competency in the domains of Engagement and Assessment; Service Planning and Delivery; Coordination, Linkage and Monitoring; Documentation; and Professional Responsibility.
The Case Manager coordinates the service delivery for individuals, and provides community partners with up‑to‑date available referral resources to provide continuity of care.
Case Managers effectively maintain and manage their assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
The Case Manager possesses a clear understanding of the service delivery system in accordance with regulations and district policy.
Case managers provide services at the time and location that is most appropriate to the person served. Travel is required.
Case Managers may also be required to conduct urine drug screening and transport clients, depending on the program.
Some programs may also require a 24‑hour on‑call rotation. Case Managers will perform other duties as assigned.
Required Education
Bachelor’s degree related to Social Work, Counseling, Psychology, Criminal Justice, Health Education, Special Education, or other human services related field.
Experience
For targeted case managers working with children: 1 year full‑time experience with children with behavioral health concerns.
For targeted case managers working with adults: 1 year full‑time experience with adults with behavioral health concerns.
Preferred Experience: 1 year of full‑time experience with the identified population.
Certification/License
BLS (Basic Life Support)
Driver’s License
CBHCM (Certified Behavioral Health Case Manager) certification from the Florida Certification Board (FCB) must be obtained within one year of employment for targeted case managers.
Preferred
Targeted case managers required to bill Medicaid.
