Location: New Port Richey, Pasco County. Status: Full-Time, Hourly. Shift Hours: Vary. Weekend work: Occasional. On Call: Yes.
The Case Manager Is Responsible For
Provide direct behavioral case management services to adults or children with serious mental health conditions, substance use disorders, and/or those involved in the child welfare system.
Demonstrate competency in Engagement and Assessment; Service Planning and Delivery; Coordination, Linkage and Monitoring; Documentation; and Professional Responsibility.
Coordinate service delivery for individuals and provide community partners with up‑to‑date referral resources to ensure continuity of care.
Maintain and manage assigned caseload, contingent on individual needs, strengths, barriers, and preferences.
Understand the service delivery system in accordance with regulations and district policy.
Provide services at the most appropriate time and location; travel required.
Conduct urine drug screening and transport clients as required by the program.
Possibly work a 24‑hour on‑call rotation and perform other duties as assigned.
Education
Bachelor’s degree in Social Work, Counseling, Psychology, Criminal Justice, Health Education, Special Education, or other human services related field.
Experience
Targeted case managers working with children: 1 year full‑time experience with children with behavioral health concerns.
Targeted case managers working with adults: 1 year full‑time experience with adults with behavioral health concerns.
Preferred: 1 year of full‑time experience with the identified population.
Certification / License
BLS (Basic Life Support)
Driver’s License
CBHCM Certified Behavioral Health Case Manager certification (Florida); must be obtained within one year of employment.
Preferred
Targeted case managers required to bill Medicaid.
