Under the supervision of the Critical Time Intervention Team Leader, the Critical Time Intervention Care Manager is responsible for providing time-limited evidence-based practice of specialized case management interventions to society’s most vulnerable individuals during times of transition.
The Care Manager links individuals to longer-term professional and community-based supports that will continue after the service has been completed.
In this role, you will:
Assess for and provide linkages to the appropriate supports.
Identifies methods to maximize independent living skills.
Assist in accessing benefits and appropriate formal services.
Assist in identifying and linking to informal community supports, such as social networks and improved family relationships.
Reduce frequency and duration of hospitalizations as well as frequency of emergency department visits & utilization of crisis services.
Provide continuity of care regardless of life circumstances or recovery environment.
Promote harm reduction, linkage to recovery treatment, and support sustained recovery maintenance.
Evaluate client needs in order to provide a personalized plan.

We require:
Bachelor's Degree in Human Services or a related field.
At least 1 (one) year of experience working with individuals with Intellectual and Developmental Disabilities and/or Mental Health Diagnoses (I/DD) preferred.
CTI certification preferred.
Knowledge of OMH and OPWDD practices, policies and regulations preferred.
Computer literacy; familiarity with Microsoft Office Suite.
Must be efficient, well-organized, and have excellent communication skills.
