As the

Health Education, Prevention, and Treatment Coordinator

, you will play a pivotal role in individuals' lives, making a significant impact through:

Review completion of internal controls according to state and agency guidelines

Perform HIV/HCV screening and testing in accordance with state, agency guidelines, and contract guidelines

Provide Hepatitis C prevention materials to at risk populations, including safer injection and safer smoking materials

Awareness of the New York State Department of Health Hepatitis C elimination plan and ability to implement aspects of the plan aspects of plan Provide follow up to individuals who complete Hepatitis C treatment with the agencies Clinical Director

Evaluate program including overall testing numbers, clients successfully receiving care for HIV, and additional education and risk reduction services including, but not limited to Syringe Exchange and PrEP

Minimum Salary: $58,500.00

Schedule: 8:30 - 4:30, 5 days a week

Qualifications

Bachelor's Degree in a related field of study

Education requirements may be waived by the agency if candidate demonstrates more significant experience (five or more years) in outreach, case management, care coordination or case management along with significant continuing education/training hours during previous employment (minimum of 35 hours a year). Plan for continued formal education is preferred, to work towards degree as stated above.

Familiarity with and sensitivity to specialized needs of people with chronic illnesses, HIV/AIDS, HCV, of People Who Use Drugs, mental health, of people of color and of GLBT individuals

Ability to work with people of diverse backgrounds

Knowledge of substance use/abuse, mental health, chronic illnesses and ability to research and seek out resources to provide clients with appropriate information

Valid NYS Driver's License and reliable vehicle, incluing car insurance and registration
