Hospice Admission Nurse Schedule Full-Time, Monday-Friday (11am - 8pm)
Job Overview As an admission nurse, you'll conduct comprehensive patient assessments, facilitate seamless admission procedures, and deliver compassionate care coordination, ensuring the best support for hospice patients and their families.
Responsibilities
Coordinates and oversees all aspects of new patient admissions
Collaborates with patient care teams to ensure seamless communication
Educates families on patient condition, needs, and care
Performs head-to-toe physical assessments
Timely and accurately completes documentation
Other duties as assigned
Requirements Education:
Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Licensure:
Current State license as a Registered Nurse; Current driver's license. Current CPR Certification.
Experience:
Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice.
Skills:
Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations. Good interpersonal skills.
Transportation:
Reliable transportation and valid auto liability insurance.
