Northway Health and Rehabilitation
specializes in providing a "home-like environment" with professional nursing and therapy services for both long‑term care and rehabilitation. This means you’ll work with residents over an extended period, allowing for deeper relationships and a more comprehensive understanding of their needs.
Position Overview Are you looking for a healthcare career that focuses primarily on patient assessment with a Monday through Friday schedule? The Resident Assessment Nurse (RN) position leads an interdisciplinary team in performing comprehensive assessments and developing care plans. The role requires excellent documentation, assessment skills, and superior critical thinking.
Responsibilities
Coordinate the interdisciplinary assessment process for assigned residents of the facility.
Coordinate development of and ensure completion of Comprehensive Care Plans in accordance with company, state and federal guidelines.
Monitor residents to ensure individual Care Plans are followed and updated as necessary; bring any variances to the Director of Nursing and Care Plan Team.
Qualifications
Must be a RN (Registered Nurse) with a required State license in good standing.
MDS experience preferred but not required; training will be provided.
Knowledgeable in general nursing standards of care and medical practices.
Excellent analytical and deductive reasoning skills.
Computer literacy and comprehensive understanding of electronic documentation programs.
