Summary Statement As a Health/Human Service Case Manager I, you will determine initial and ongoing eligibility for all needs‑based benefits, including SNAP, Medical Assistance, CHIP, subsidized childcare, and State Funded General Assistance (GA). You will also work closely with SNAP vendors, provide entry‑level counseling, case management, and referrals for benefits not offered by DSS (such as child support, foreclosure assistance, or food closets). Your role is to fulfill the diverse needs of vulnerable and low‑income residents by providing timely, accurate information about benefits.
Essential Functions Essential functions are core duties common to all positions in the class and are not exhaustive. This class is distinguished from a licensed social worker; employees do not diagnose disorders, conduct psychotherapeutic treatment, or develop assessment‑based treatment plans.
Interview clients, family members, and others to determine eligibility and connect clients with health and human services.
Develop a service plan to aid and assist a client in receiving care and assistance needed in collaboration with hospitals, clinics, agencies, and other appropriate resources.
Evaluate information derived from client interviews, case history, and other pertinent sources and refer to proper channels for review.
Provide guidance and relevant information to clients and families prior to receiving requested service and assistance.
Coordinate with other agencies, hospitals, clinics, and resources to expedite the process on behalf of the client.
Prepare case histories and reports and maintain records and chronological logs of case activities.
Use automated information systems and computer software to research, review, evaluate, and report on health/human service data.
Communicate and interact effectively with people across cultures, ranges of ability, genders, ethnicities, ages, and races.
Perform other related duties as required.
Job Requirements
Possession of an Associate’s degree or higher in Behavioral or Social Science or related field.
Or three years’ experience in health or human services support such as interviewing clients and assessing personal, health, social or financial needs in accordance with program requirements, or coordinating with community resources to obtain client services.
Six months’ experience in making recommendations as part of a client’s service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
Six months’ experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.
Six months’ experience in narrative report writing.
Additional Posting Information The State of Delaware recognizes the value of telework and flexible working schedules. This position is eligible for telework after the probationary period as per the State of Delaware’s Alternative Work Arrangement Policy. More information will be shared at the time of interview.
Conditions of Hire Applicants must be legally authorized to work in the United States.
