Coverage Area: Broward and North Dade Counties

Salary:
DOE $25-30 hr plus mileage outside Broward County
Schedule:
3/12s 7p-7a / rotate weekends

What You Need To Know:
Must hold a current LPN license in the practicing state(s); an associate degree is preferred. CPR certification is required for direct patient care in Georgia and preferred elsewhere.
At least one year of nursing experience in hospice, home health, or aligned patient care is required (e.g., geriatrics, pediatrics, med‑surg).
Strong written, verbal, and organizational skills, with proficiency in Microsoft Office and EMR systems.
A valid driver's license, reliable vehicle, and meeting minimum state‑required auto insurance are essential.
Ability to work within an interdisciplinary team and knowledge of relevant state, agency, and federal hospice policies and protocols.
This role involves physical demands, including frequent lifting (up to 50 lbs), assisting patients (up to 200 lbs), walking, driving, and computer use, with occasional exposure to varying environmental conditions.
Your Responsibilities:
Deliver skilled nursing care to patients and families, following the interdisciplinary plan of care – assess, plan, implement, and evaluate effectively.
Coordinate with care teams, physicians, and external partners to ensure exceptional continuous care while maintaining updated patient records.
Educate caregivers about medication management, personal care, mobility assistance, end‑of‑life preparation, and other essential aspects of care.
Communicate effectively with the Director of Clinical Operations and the interdisciplinary group (IDG) regarding any complex patient or family matters.
Provide compassionate support during critical moments, including at the time of death, while contributing to 24/7 care coverage as required.
Engage in team meetings, training programs, compliance efforts, and assist with additional tasks within your professional licensure scope.
