About the job: The Registered Nurse under the direction and supervision of the Clinic Supervisor provides skilled nursing care to participants, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family teaching, and communicates participant’s condition to primary caregivers, physician and the Interdisciplinary Team.

Required Qualifications:

Graduate of a school of professional nursing

Current state Registered Nurse (R.N.) license

Cardio Pulmonary Resuscitation (CPR) certification required

A minimum of one year’s experience working with the frail or elderly population required

Must have a valid driver’s license, proof of insurance and reliable transportation

Must have medical clearance for communicable diseases and up‑to‑date immunizations before direct participant contact

Preferred Qualifications:

Bachelor of Science Nursing (BSN) preferred

Essentials:

Conduct in‑person comprehensive initial nursing assessments with new PACE enrollees and coordinate with the Interdisciplinary Team to develop a comprehensive plan of care for each participant

Conduct in‑person nursing re‑assessments semi‑annually and as needed

Serve as a member of the Interdisciplinary Team, maintaining regular attendance at and participating in Interdisciplinary Team meetings, communicating participant changes, collaborating on plan of care decisions and coordinating for 24‑hour care delivery

Provide orientation, initial and annual competencies for Licensed Practical Nurse (LPN), Certified Nursing Assistant (C.N.A.) and Clinic Clerk staff as needed

Perform competencies on appropriate day‑center staff as needed

Perform general nursing care and supervise administration of prescribed medications and treatments in accordance with nursing standards

Triage participants to determine priority of care
